Castle Connolly publishes a book and an online service called
America's Top Doctors
http://www.castleconnolly.com/doctors/index.cfm

Doctors are selected by other doctors with the question, "Who would you send your family member to for [specific procedure] "

You can do a free search, and using fewer words works better -- use "bypass" not "heart bypass."
Purchasing a premium subscription (@$25/yr) retrieves more information.

The book comes out annually, usually in February (wait for the 8th ed) and can be ordered on Amazon.com for around $20.
